Proof of Name Change Requirements
If you've changed your name due to marriage or for other reasons, you'll need to update your passport to reflect your current legal name. To do this, you'll need to apply for a Passport Name Change to update it.
You’ll need to provide provide one of the following forms of proof to support application for a passport name change:
Original Marriage Certificate / Original Divorce Decree
